Coordinating the swallow with the expiratory phase of breathing is the safest approach for a ventilator-dependent patient with a tracheostomy. Presenting the bolus at the beginning of exhalation aligns the swallow with a period when the airway is being protected by an expiratory airflow and the laryngeal mechanism is ready to seal. This timing helps keep material out of the airway and allows any residuals to be cleared by the subsequent exhale, which is especially important when airway sensation and protection may be altered by the tracheostomy. Swallowing during other parts of the respiratory cycle—such as during inhalation or later in the exhale—carries a higher risk of penetration or aspiration. Keeping therapy ongoing with this timing supports safety while working on swallowing skills, rather than defaulting to NPO.
